Benign paroxysmal positional vertigo (BPPV) is the most common disease of the peripheral vestibular disorder. Recently, many cases of posterior and lateral canal BPPV were reported. But there are few cases about superior canal BPPV, particularly about cupulolithiasis. The incidence of superior semicircular canal BPPV was reported about 3%. We report a 49-year-old man who showed counter-torsional downbeat nystagmus in left side Dix-Hallpike position. The nystagmus was continued for 3 minutes and the symptoms were eventually relieved after reversed Epley maneuver. Key Words : Cupulolithiasis, superior semicircular canal
