  • A 84-year-old woman presented with a two weeks history of dizziness, slurred speech and ataxia. The neurological exam showed spontaneous left beating nystagmus, horizontal gaze evoked nystagmus and limb ataxia. A few weeks earlier, she had an upper airway infection. Brain MRI revealed diffuse leptomeningeal enhancement in the both cerebellar hemisphere and brain single photon emission computed tomography (SPECT) showed hyperperfusion in both cerebellar hemisphere. Extensive laboratory studies with cerebrospinal fluid analysis did not reveal any etiologic factors. She was started on methylprednisolone (1 g/day for 7 days), gradually improved over the weeks. Post infectious cerebellar ataxia is a neurologic complication that occasionally follows systemic viral and bacterial infections. This case demonstrates that cerebellar abnormalities can be detected by brain magnetic resonance imaging and SPECT.
감염 후 소뇌실조(post infectious cerebellar ataxia)는 주로 비특이적인 바이러스 감염 이후에 안진(nystagmus), 구음장애(dysarthria), 측정 이상(dysmetria) 등의 소뇌이상을 보이는 질환으로 소뇌염(cerebellitis), 바이러스감염 후 소뇌실조(post-viral cerebellar ataxia)와 동일한 의미로 사용되어 지기도 하며 예후는 비교적 나쁘지 않다[1,2]. 뇌 컴퓨터단층촬영(computed tomography, CT)과 뇌 자기공명 영상(magnetic resonance image, MRI)은 정상 소견을 보이는 것이 일반적이지만, 드물게 뇌 MRI의 T2 강조영상에서 양측 소뇌에 대칭적인 조영증강이 관찰 될 수 있다[3]. 하지만 촬영시기에 따라서 조영증강이 보이지 않을 수도 있으며 뇌MRI에서 보이는 이상 소견과 증상과의 상관 관계는 명확하지 않다[4,5]. 최근에는 뇌 MRI에서 정상을 보인 감염 후 소뇌실조 환자에서 뇌 단일광자방출컴퓨터단층촬영(single photon emission computed tomography, SPECT)을 통해 양측 소뇌의과관류(hyperperfusion)를 확인하는 것이 진단에 도움이 된다는 보고가 있다[6,7]. 저자들은 뇌 MRI와 SPECT 모두에서 이상 소견을 보인 감염 후 소뇌실조 환자를 경험하였기에 보고한다.
고지혈증 외에는 특이병력이 없는 84세 여자가 7일 간의 어지럼과 하루 동안의 보행장애로 왔다. 환자는 한달 전에 기침, 콧물 등의 상기도 감염 증상이 있었으나 특별한 치료없이 경과관찰을 하였으며 일주일 후 증상은 저절로 호전되었다. 그 후 7일 뒤 복시증상이 발생하여 안과외래에 방문하여 눈 MRI를 촬영하였고, 8일 후부터는 어지럼이 발생하여 점점 심해지고 보행이 어려워졌다. 내원 당시 활력징후는 정상이었지만, 시행한 신경학적 검사에서 제일안위(primary position)에서 왼쪽으로 향하는 자발 안진을 보였으며, 좌우 수평방향으로 주시를 하였을 때 주시 방향에 따라 방향이 바뀌는 수평적 주시유발안진(gaze evoked nystagmus, GEN)이 관찰되었으나 두부충동검사(head impulse test, HIT)는 정상이었다. 또한 환자는 불분명발음(slurred speech)를 보였고 근위약은 없었으나 주변의 도움 없이 혼자 보행이 불가능 하였으며 손가락코검사(finger to nose test), 발꿈치정강이검사(heel to shin test), 상반운동 반복검사(diadochokineisa test)에서 좌우측 모두 이상 소견을 보였고 좌측이 우측에 비해서 좀 더 심한 양상이었다. 사지의 근력과 감각은 정상이었으며, 심부건반사(deep tendon reflex, DTR)도 정상이었다. MRI를 확인하였고 양쪽 소뇌피질 부위의 경막조영증가와 부종(edema)이 관찰되었다(Fig. 1). 뇌척수액 검사는 white blood cell 3개를 보였고 단백과 당수치는 정상이었다. 단순헤르페스바이러스(herpes simplex virus, HSV) I, II 형과 수두대상포진바이러스(varicellar zoster virus, VZV) immunoglobulin M (IgM)과 중합효소연쇄반응(polymerase chain reaction, PCR)은 음성이었고 결핵과 관련된 acid-fast bacilli (AFB) 염색과 배양, 그리고 PCR도 음성이었다. 크립토코쿠스 항원 역시 음성이었으며 어떠한 곰팡이(fungus)도 배양되지 않았다. 혈액검사에서도 HSV IgM, VZV IgM, 엡스타인-바바이러스(Epstein-Barr virus, EBV) IgM은 음성이었고, Miller-Fisher증후군이 배제되지 않아 시행한 항 GQ1 항체 검사도 음성이었으며, 신생물 딸림증후군(paraneoplastic syndrome)의 가능성을 배제하기 위해 시행한 가슴 및 복부CT, 종양표지자검사(tumor marker). 신생물딸림항체(paraneoplastic antibody)인 항 Hu 항체, 항 Yo항체, 항 Li 항체의 검사도 정상이었다. 뇌 SPECT를 시행하였으며 소뇌의 관류가 증가된 것이 확인되어 최종적으로 감염 후 소뇌실조로 진단하였다(Fig. 2). 스테로이드 정맥주사 부하 치료(steroid pulse therapy)로 메칠프레드니손(methyprednisolon) 1,000 mg을 사용하였으며, 치료 후 환자증상은 조금씩 호전되어 8주 후 보행이 가능한 상태로 재활의학과로 전과되었다.
감염 후 소뇌실조는 비특이적인 바이러스 감염 이후에 동반될 수 있는 양성 질환이며 대게는 좋은 예후를 가지고 회복한다[1]. 또한 이전에 연구된 사례들을 보면 뇌 MRI에서 나타나는 병변이 가역적이고 시간이 지나면 증상과 함께 영상학적 소견도 호전된다고 알려져 있다[1,2]. 감염 후 소뇌실조 환자가 뇌 MRI에서 이상 소견을 나타낸 경우는 약 5%에서 보고되고 있으며, 대부분 다음과 같이 나타난다고 한다. 첫째, 양측 소뇌의 부기 및 비정상적 연수막 증강을 보일 수 있으며, 둘째, 소뇌의 부기 및 소뇌 피질의 비정상적 고신호강도가 나타나고, 마지막으로, 양측 소뇌 반구 및 소뇌다리 백질의 고강도신호를 특징적으로 보일 수 있다[1,3,4]. 소뇌에 비정상적인 강도가 나타나는 기전으로는 염증이나 뇌염, 소뇌 혈류의 변화로 인한 혈액-뇌 장벽의 손상 때문이라고 알려져 있다[1]. SPECT는 단층 영상을 얻을 수 있어 병소를 정확히 찾을 수 있으며 임상적으로 이용되는 SPECT는 뇌관류, 심근관류, 뼈, 신장에 대한 부분이 많다. 특히 뇌관류 SPECT는 혈류에 비례해서 뇌 조직에 섭취되는 방사성 의약품을 이용하여 뇌 조직으로 공급되는 혈류량을 직접 평가할 수 있어 국소 뇌 관류의 이상을 평가하는데 주로 이용된다[8,9]. 감염 후 소뇌실조를 나타낸 환자들에게서 뇌 SPECT를 시행한 증례보고는 있었으나 체계적인 연구 및 통계자료는 부족하다[4]. 증례 보고에 따르면 양쪽 소뇌의 과관류 혹은 저관류 모두가 나타날 수 있는데 과관류의 기전은 소뇌의 염증에 의해 해당 병변으로 혈류량의 증가에 의한 결과라고 보고 되고 있다[7]. 반대로 저관류의 기전은 감염 이후 발생한 자가면역적 반응으로 오히려 혈류의 흐름이 감소되어 나타난다는 설명이 있었다[7]. 저자들의 증례에서는 뇌 SPECT를 시행하였고, 양쪽 소뇌의 고관류를 확인하여 염증에 의한 혈류량의 증가로 인해 생긴 것으로 생각된다. 소뇌실조 환자가 증상발생 시에 시행한 뇌 MRI는 실제 이상 소견을 확인할 수 있기는 하지만, 증상호전과 함께 영상 소견 또한 호전된다고 알려져 있고, 증상의 정도 및 영상 소견이 예후와 비례하지 않는다[9,10]. 실제로 뇌 MRI에서 정상인 경우에도 사망하거나 보행장애와 같은 후유증이 발생한 경우도 있으며, 반면에 매우 특징적인 MRI 소견을 보였지만 완전히 회복된 경우도 있었다[6]. SPECT는 증상 발생 2주 이내에 시행한 경우에 소뇌의 과관류 소견이 대부분 확인되고, 추적검사로 증상 호전과 함께 영상의 호전 소견을 보인다는 점에서 MRI에서 병변이 확인되지 않은 감염 후 소뇌실조 환자의 진단에 유용할 수 있다. 저자들은 소뇌의 고관류를 확인할 수 있는 SPECT의 시행으로 병변을 확인하였으나 다른 증례와 달리 추적 SPECT를 시행하여 환자의 증상과 영상학적 호전을 확인하지 못한 점은 이전 연구된 증례와의 차이점이자 한계점으로 볼 수 있으며, 또한 현재 SPECT검사의 적절한 시기 및 증상 이 호전되었을 경우 추적검사의 시기나 병의 심각도가 실제 영상과 어떤 관련성을 가지는가에 대해서는 추후 연구가 필요할 것으로 생각한다.
