  • Traumatic brain injury (TBI) could give rise to variable clinical manifestations based on the involved structures of our bodies. Although there are no structural abnormalities proven, the patients with mild TBI suffer from chronic dizziness and imbalance. Herein, I will discuss the visuo-vestibular interaction and neurotological finding in TBI, which could demonstrate the clue to the diagnosis and management in dizzy patients with TBI.

외상성뇌손상은 그 손상 정도에 따라 경도(mild), 중등도(moderate), 중증(severe), 그리고 회복 결과에 따라 회복(good recovery), 중등도 장애(moderate disability), 심한 장애(severe disability), 지속적 식물 인간 상태(persistent vegetative state), 그리고 사망(death)으로 분류한다[1]. 특히, 경도외상성뇌손상(mild TBI)은 뇌진탕(concussion)과 혼용되는 용어로, 대개 7–10일 안에 저절로 호전되는 경과를 보인다고 알려져 있으나, 이학적 검진이나 뇌 computed tomography (CT), 뇌 magnetic resonance imaging (MRI) 등에서 뚜렷할 만한 이상이 없으나, 지속적으로 어지럼과 균형이상을 호소하는 경우를 드물지 않게 볼 수 있다. 그러나, 경도 외상성뇌손상을 객관적으로 진단할 수 있는 표준진단법이 아직 정립되지 않아, 관련 환자를 진단, 치료하는 과정이 쉽지 않다.
중등도, 중증 외상성뇌손상의 경우 국소적인 신경학적 이상을 보이는데 반해, 경도의 경우 미만성대사이상과 축삭손상으로 인해 다양한 증상을 보인다. 따라서, 환자를 제대로 이해하기 위해 주관적 증상 이외에, 인지, 심리, 운동, 균형, 보행 등 객관적 검사가 필요하다[2]. 어지럼은 외상성 뇌손상 이후 발생하는 매우 흔한 증상으로, 수상 초기 대부분의 환자(23%–81%)가 호소하는 것으로 알려져 있다. 두부손상 이후 발생하는 전정기관이상은 측두골 골절, 미로좌상, 양성돌발두위현훈, 혈관이상, 중추성병변, 외림프누공 등이다. 전정기능검사상 각전정안반사 이득 감소와 전정척수반사이상이 관찰될 수 있다. 이외에도 불안, 전정편두통, 자율신경조절이상, 경추성 지럼이 외상성뇌손상 후유증으로 발생할 수 있다.
외상성뇌손상 환자 검진은 비외상성 환자와 유사하며, 이외에도 동반 질환에 대한 자세한 평가가 수반되어야 한다. 주관적인 증상 평가를 위해 흔히 사용되는 설문지는 어지럼장애척도, Korean version of Activities-specific Balance Confidence Scale 등이다. 시각움직임에 의해 유발되는 증상은 관련 설문지(Situational Vertigo Questionnaire or the Visual Vertigo Analog Questionnaire)로 평가할 수 있다. 이때, 개개인의 필요와 목적을 적절히 고려할 수 있는 설문지를 선택하는 것이 필요하다. 예를 들어 운동 선수의 경우 이전에 본인이 하던 운동을 가능하게, 일반인의 경우 병전 일상 생활을 가능하도록 하는 과정이 궁극적으로 필요하기 때문이다.
체위조절 능력은 전산화동적자세검사(computerized dynamic posturography)나 체간조절감각자세유지검사(modified Clinical Test of Sensory Interaction in Balance)를 이용해 측정한다[3,4]. 환자의 보행은 기능보행평가(Functional Gait Assessment, FGA), 동적보행지수(dynamic gait index) 등을 이용해 평가한다. FGA 점수는 22점 이상인 경우 낙상 위험이 높다고 알려져 있다(30점 만점). 전반적 균형을 평가하는 데 유용한 방법은 mini-Balance Evaluation System Test (Min-BESTTest)다(www.bestest.us/index.php/download_file/view/18/135/).
전정안반사 기능을 평가하기 위해 동적시력검사를 이용하는데, 정적, 동적 시력의 차이로 계산한 logMAR 점수(log minimal angle of resolution)로 보고한다. 최근 도입된 전산화 방법이 경도 외상성뇌손상 환자의 주시 안정성 이상 여부를 확인하고 추적 관찰하는데 유용하다고 알려져 있다[5]. 이외에 외상으로 인한 안구운동이상이 어지럼의 원인이 될 수 있으므로, 그 기능을 정확히 평가하고, 교정해야 한다.
경도 외상으로 인해 뇌손상이 발생하는 경우 일주일이 경과하면[6] 여러 임상적 평가법의 민감도는 급격히 떨어져, 환자의 증상 지속에도 불구하고, 건강상태로 섣불리 진단할 수 있다. 특히 경한 두부외상의 경우 뇌 CT나 MRI상 이상이 없으나, 확산텐서영상(diffusion-tensor neuroimaging, DTI)상 회백질이상과 미만성축삭손상이 관찰되어 신경기능이상을 알 수 있다[7-10]. 특히, 전정 증상을 호소하는 환자의 경우 중추전정계 침범을 시사하는 소뇌 DTI 이상이 관찰되었다[11]. 해부생리학적으로 이들 이상은 대뇌반구, 소뇌, 뇌간을 포함하는 중추신경계회로 이상을 시사하기 때문에, 경도 외상 환자의 증상을 이해하기 위해 안구운동과 신경이과적 접근이 임상적으로 매우 중요하다(Table 1) [12-15]. 이를 확인하기 위해 따라보기(smooth pursuit), 의도적 신속보기(volitional saccade), 안진을 검사한다. 전정안진 고정 억제(fixation suppression)는 소뇌, 전정핵 사이 신경로를 반영하므로, 중추전정손상, 시각-전정 상호작용(visuo-vestibular interaction)을 평가하는데 이용될 수 있다. 특히, 전정 자극 시 유발되는 최고 속도 안진은 시선고정에 의해 50% 이상 감소하지 않으면, 두정-후두 피질, 뇌간, 소뇌를 포함하는 중추신경병변을 시사한다. 최근 가상현실을 이용한 연구에서 경도 외상성뇌손상 환자의 공간 및 자가 운동(self-motion) 지각은 시각-전정 부조화(abnormal visuo-vestibular processing)에 의해 영향을 받는 것으로 나타났다. 또한, 경도외상 후 제반 증상이 호전되는 7–10일이 경과하여도, 시각-전정 부조화는 지속되어, 환자가 호소하는 어지럼, 균형이상, 시각 불편감 등을 부분적으로 설명할 수 있다[16].
시각적 움직임을 처리하는 과정은 자가 운동 지각, 개체와 환경의 움직임을 구분하는데 필수적이며, 전정, 고유감각, 체성감각, 시각 등 다중 감각 비교를 포함한다[17,18]. 시각과 전정기능 처리에 관여하는 신경 상호작용은 행동, 영상연구를 통해 알려졌다. 두정-후두엽과 두정엽섬전정피질(parietal insular vestibular cortex)은 상호 연결되며, 양측 전정신경핵에 교차 연결되어 체위조절, 지남력, 운동지각에 관여하는 것으로 알려져 있다[19,20]. 전정계는 두부움직임 신호를 뇌로 보내어 전정안반사와 전정척수반사를 통해서 주시와 체위를 안정화시키는 역할을 한다. 또한, 전정계는 효과적인 체위 균형과 공간 지남력 유지를 위한 반사와 지각 기능을 담당한다. 뇌는 시각, 전정감각, 고유감각 정보를 통합하여, 자신과 주변 움직임을 구분한다. 이들 신호가 상호 보완적이라면 동작 인지가 정확하나, 시각 자극과 전정신호 사이에 부조화가 발생한 경우 움직임을 적절히 인지하지 못한다. 한 연구에 의하면, 시각-전정 정보가 일치하지 않는 경우 후섬이랑(posterior insular gyrus)과 가로측두(transverse temporal areas), 소뇌편도(cerebellar tonsil), 띠다발이랑(cingulate gyrus)과 내이마이랑(medial frontal gyrus)의 활성이 증가하는 것으로 나타났다[21]. 반대로, 시각-전정 정보가 일치하는 경우 일차/이차 시피질이 활성화되었다[21]. 이는 움직임에 대한 정보가 일치하지 않는 경우, 시각/전정 감각 통합을 위해 후섬이랑 등 다감각 전정 피질이 활성화됨을 시사한다[21]. 가상현실을 이용한 체위검사 결과, 외상 후 시각-전정 통합과정에 어려움을 겪는 것으로 나타났다[22]. 이는 매우 경미한 외상 이후에도 관찰되며[23] 이를 치료하기 위해 시운동자극/습관, 시/육체적 요동자극, 광범위시야자극 등의 다양한 환경을 체위 안정 운동 시 역이용할 수 있다. 즉 다양한 시각/전정자극 환경에서 체위안정운동시 증상 호전을 보이므로, 외상성 어지럼 환자에서 맞춤 전정 운동의 방법으로 고려할 수 있다[24]. 많은 어지럼 환자는 일상 생활에서 시각 자극에 노출되거나, 움직일 때 어지럼을 자주 호소하는데, 이는 시각-전정 피질 상호작용의 부조화가 원인으로 보인다[25,26]. 이때, 과거 연구에 의하면, 전정기능 역치는 정상보다 증가될 수도 있고, 감소될 수도 있다. 전자에 대한 설명은, 주변 시각 자극에 과민해지면, 일반화 효과(generalized effect)를 초래하여, 전정 역치 또한 감소한다는 설명이다. 그러나, 일부 연구에서는 이와 상충된 효과를 주장하는데, 전정피질 신경망의 잡음이 더 증폭되어 마치 이명(vestibular tinnitus)과 유사하게 작용한다는 것이다[27]. 이는 어떠한 결정에 소요되는 시간(내가 어느 방향으로 이동하고 있는가?)을 예측하는데 이용하는 Drift Diffusion model을 적용하여 개념화할 수 있다[28,29]. 즉, 감각 정보 분석 시, 부적절한 감각 잡음이 더해지게 되면, 의사 결정 과정이 늦어지게 되고, 결과적으로 동작을 감지하고, 그 방향성을 인지하는데 더 많은 시간이 소요된다는 설명이다. 즉, 다양한 원인의 현훈 환자에서 시각-전정신경망 잡음(noise)가 증가하여, 동작 감치 역치가 상승한다는 설명이다[28].
외상 이후 뚜렷한 증후나 뇌CT나 MRI에서 구조적인 이상이 없으나, 지속적으로 어지럼과 균형이상을 호소하는 환자를 흔히 볼 수 있다. 이들을 대상으로 시행한 신경기능 영상연구 결과 신경망 회로의 이상이 관찰되었다. 외상후 만성 어지럼을 호소하는 환자의 공간 및 자가 운동(self-motion) 인지능력은 시각-전정 상호 부조화에 의해 영향을 받는 것으로 나타났다. 이러한 현상은 경도의 외상후에도 관찰되어, 외상후 만성어지럼을 이해하는데 도움이 되었다. 향후 기능적 평가에 근거한, 맞춤전정운동 및 신경조절 등 치료에 대한 연구가 더욱 필요하다고 본다.
